This notice explains how Physiolife handles the information you share through our patient feedback page. We process your information in line with the Protection of Personal Information Act, 2013 (POPIA).

We only ever collect what you choose to share. Providing your name or contact details is optional.
If you rate us 4 or 5 stars, we invite you to share a review on Google. That is entirely voluntary. A Google review is public and is governed by Google's own terms and privacy policy — it is separate from this feedback page.
We process the feedback you submit on the basis of your consent, which you give by submitting the form. You may withdraw your consent or ask us to delete your feedback at any time using the contact details above.
Your feedback is private to the Physiolife practice team. It is stored securely with our database provider (Supabase) and is not sold, published, or shared for marketing. We may share information only where the law requires it.
We keep your feedback for up to 24 months to address your concern and improve our services, after which we delete or anonymise it.
Under POPIA you may ask to access the information we hold about you, correct it, or have it deleted, and you may object to how we use it. Contact our Information Officer to do so. If you believe we have not handled your information properly, you may lodge a complaint with the Information Regulator (South Africa) — inforegulator.org.za.
